You need to complete an application and provide some documentation so we can see if your children qualify
For most families CHP+ is free. Depending on your monthly income, you may have to pay a small enrollment fee and small co-payments when you receive services and prescriptions. Click here to learn more about the enrollment fee and co-payments.
If you or your child qualifies for CHP+, coverage will begin on the date your application was received at the CHP+ office, a CBO Site, or at a county office. Once you or your child is approved, you will receive information in the mail including a membership handbook and membership card.
Here are just a few of the health services CHP+ covers:
CHP+ works with Health Maintenance Organizations (HMOs) to provide medical care. Each HMO has their own network of doctors that your child can see on CHP+. The county you live in will determine which HMO your child enrolls with. If more than one HMO is available in your county, you must select an HMO. Click here for more information about HMOs.
CHP+ dental services are provided by Delta Dental of Colorado dentists. To find a dentist in your area, click here.
You may qualify for a program called Presumptive Eligibility (PE). PE is a way for children and pregnant women to receive care right away without having to wait to see if you are eligible. To apply, you must visit a PE Site and they will do a quick review. If found eligible you will get enrolled for up to 60 days while you wait to see if you qualify. Click here to find a PE Site in your area.
You can call CHP+ Customer Service at (800) 359-1991. We have representatives ready to assist you! CHP+ Customer Service is available Monday through Friday, 8:00 AM to 6:00 PM (se habla español). You can also visit our office. For directions and more contact information, click here.
CHP+ has a wide range of free materials for distribution that contain helpful information for families interested in the programs. CHP+ offers applications, brochures, and posters for distribution. Click here to place an order.
As of April 2013, CHP+ enrollment is 71,206 children and 1,144 pregnant women.*
